Application and Reports
The My Stuff Bags Foundation invites your organization to apply for My Stuff Bags for the children you serve. To apply please follow the guidelines below:
My Stuff Bags Application:
- You must be a 501 (c) (3) organization or other non-profit status agency. Please furnish a copy of your Federal verification of non-profit status along with your application.
- Fill out the My Stuff Bags Recipient Application and make sure it is signed by a Director or Senior Manager.
- Submit to us by mail or submit via email at info@mystuffbags.org.
My Stuff Bags Recipient Information Report:
- 2 months after receiving your My Stuff Bags, you must fill out and return our My Stuff Bags Recipient Information Report even if you have not yet distributed all the bags awarded to you. This is a requirement to receive My Stuff Bags in the future.
- Fill out the My Stuff Bags Recipient Information Report and submit via email to info@mystuffbags.org
- We cannot accept faxes!
